Combined treatment of chemioexfoliation, photosensitizing photodynamic therapy and non-ablative laser in adolescent patients with acne

Abstract

Acne is a dermatosis of the pilosebaceous unit that affects 85% of adolescents. Due to the indiscriminate use of topical and oral treatments, a decrease in their effectiveness has been evidenced. This is how the present work proposed the use of combined treatment with Chemoexfoliation, Photodynamic Therapy with photosensitizing agent and non-ablative laser, in order to evaluate the effect of a therapeutic alternative for acne. A case report was made with 4 patients in the aesthetic medicine clinic of the state of Miranda, Caracas, in a period of 3 months, with the following inclusion criteria: mild, moderate or severe acne, ages between 18-24 years, without treatment with Isotretinoin at the time of the study, informed consent and photographic record before and after treatment. Diode laser with triple wavelength 755nm, 808nm and 1064nm was applied with cooling system, pulsed, fluence between 8-10 J/cm2, frequency of 3-4Hz and pulse duration of 35ms, in addition photodynamic therapy and ALA as a photosensitizer. A photodynamic lamp with wavelengths of 640nm, 400nm and 490nm was applied weekly. The mean age of the participants was 20.5 years. Subsequently, satisfactory results were evidenced with a decrease in the number of lesions, inflammation level, sebum production and skin improvement, with desquamation and erythema being the adverse effects reported in half of the sample. Complications will not occur and it will be interrupted that the combined application of such treatments is safe and effective in acne.
